You will need to download and complete the Health History Form & Consent/ Policy form & Body Map prior to your first session. If unable to do so, please notify me as soon as possible. I can leave the paperwork for you in the lobby. Give yourself at least 10 minutes before your scheduled appointment to compete all the forms.
